Participant Waiver and Consent Form

Assumption of Risk
I acknowledge and understand that participation in activities at Swing Sportsplex, including but not limited to simulator and batting cage use, pitching, fielding, and training exercises, involves inherent risks. These risks include, but are not limited to, being struck by balls or bats, pitching machine errors, collisions, slipping, tripping, or other accidents. These risks may result in serious injury or death.
By signing this waiver, I certify that participation is voluntary and that I assume all risks, known and unknown, associated with participation.
Medical Consent
In the event of an injury or medical emergency, I authorize Swing Sportsplex staff to obtain emergency medical treatment for the participant as deemed necessary. I understand that I am financially responsible for all costs associated with such treatment.
Release of Liability
In consideration of being permitted to use the facilities and equipment of Swing Sportsplex, I hereby release, waive, discharge, and agree not to sue Swing Sportsplex LLC, its owners, employees, instructors, contractors, or volunteers from any and all liability, claims, demands, or causes of action arising out of or related to any loss, injury, or property damage sustained while on the premises or participating in any activity.
This release includes claims based on negligence, including but not limited to supervision, instruction, or maintenance.
Facility Rules and Conduct
I agree to follow all posted rules, staff instructions, and safety guidelines. I understand that failure to comply may result in suspension or removal from the facility without refund.
Key Safety Rules Include:
- Helmets must be worn inside batting areas
- Only one batter allowed per cage at a time
- No swinging bats outside designated areas
- Follow all staff directions at all times
Photo & Media Consent
I authorize Swing Sportsplex LLC to use photographs and/or video of the participant for promotional purposes, including social media, website content, and printed materials.
Acknowledgment and Signature
I have read, understood, and voluntarily agree to the terms of this Waiver and Consent Form. I acknowledge that I am giving up substantial legal rights, including the right to sue.
